The application form and a curriculum vitae (CV) should accompany the letter. The CV should contain, aside from the typical items, information on scholarships received, publications, and extra-curricular activities while at the University of Waterloo.

This award was established in memory of the late M. Chandrashekar, a Professor in the Department of Systems Design Engineering for more than twenty-five years. It is expected that the awardee be a full-time graduate student at the University of Waterloo at the time of application. He/she may not be beyond the third term in the Master's program or the ninth term in the PhD program.
